摘要: 全民健康保險對於國人而言是一項非常重要的政策,開辦後大幅改善了民眾醫療可近性低的問題,使有就醫需求的病患不需要再擔心因病而貧、因貧困而不敢就醫的問題,對於台灣社會整體發展有非常大的幫助,然而這項重要政策的財務狀況在近20年出現了危機,健保收入逐漸無法支應成長快速的健保支出,演變至健保年度財務入不敷出的情形,而主管機關為應對此一困境,提出了二代健保、提高健保保費費率等收入面的方式,但皆僅能維持短期的財務穩定,長期而言健保支出又會再度多過健保收入,政府面臨需一再提高民眾保費的困境,而隨著社會高齡化及少子化的情形日益加重、以及民眾收入成長低於醫療支出成長的情況下,恐難僅倚賴調整健保費率解決問題,因此若要確保台灣健保永續發展,需要提出更長遠的健保財務解決方案。\n本研究先針對台灣健保做整體分析,包括健保創立目的、健保財務收支架構及目前所面臨的問題,以對台灣健保有更全面的了解,再將此一財務問題結構化並將健保財務分成收入端及支出端建立模型,進一步分析找出可行性及效益較高的方案,發現到總額預算制中的醫院部門為較好的切入點,因此以提升醫院經營及財務表現效率為目標,分析國外醫療機構及台灣醫院在經營模式及創新發展的差異處,並且找出台灣醫院可能可以採用並且會提升其經營及財務效率的新商業模式,分析其效益及現階段醫院及主管機關需要克服的問題,最後則是針對台灣醫院所處的健保制度及社會風氣等外在環境,對於醫療機構創新的誘因做探討,同時提出若要增進醫院創新以提升其經營效率的動機,主管機關及民眾可能需要做出的改變。
