摘要:   失能長期照護及人口老化是社會重大議題,如何有效預防及治療是造就社會永續發展的根本。身體的殘障造成的失能須考量個人疾病的因素及社會環境的因素,從醫療照護系統提供疾病的診治,社會環境應提供友善及便利的環境設施,以避免疾病造成的失能。長期照顧法服務是政府推動重大政策,其服務模式、人才、資源及財源配置,更需要整合性的思維。除了提供有效便利的人力服務外,更須考量發展兼具疾病預防、提升病人及家人的幸福及滿足感,符合自然及人性的服務模式,創造價值共享的社會。\n 本文研究包含三部份,首先分析日本熊本縣之長期照顧模式,從社區日間支持照護、老人安養照護機構及區域主責醫院,質性研究探討其組織架構、服務模式、資源配置及經費來源等,並進一步思考如何架構一個屬於國內長照服務可行的照護模式。探討國內現有精神醫療體系在長照服務建立時可仿效的處遇模式。其次在醫院精神科日間照護中心建立魚菜共生系統取代傳統園藝治療,架構一個團體治療及復健的平台,質性評量精神病患治療成效,並分析其主觀感受及需求。第三部份問卷調查社區長照及綠色療癒的推廣應用。\n 研究結果顯示日本成熟的長照服務模式,國內導入時可架構在現有完備的精神醫療網進行策略規畫,為考量社區化與住宅屬性的差異,採用微生態魚菜共生系統服務模式,將其當作復健的平台,是長照服務架構一個值得思考的議題。創造出一個新的服務模式,讓老年人得以參與社區活動,接觸自然,增加活動量減少孤獨與失能,達到疾病預防及療癒的目的。論文希望以魚菜共生系統為平台,由課程的設計、從植物種植與觀察生長歷程、至成果分享與交換,重拾失能者與老年人的生產力,由此創造更多親屬及朋友間的互動平台,甚至創造更多的就業機會,讓年輕人投入長照服務產業,創造世代間的共享價值。
