Spatial Regression Model (SLM&SEM)日期 2024 上傳時間 4-Sep-2024 14:27:18 (UTC+8) 摘要 本研究根據衛生服務利用理論，建構「可及性」為衡量在地老化設施影響力之因素。有別於過去多以問卷調查資料進行研究，本研究率先採用母體資料，並以量化迴歸之方式進行實證，將設施可及性作為代理變數，探討在地老化設施對老人行動健康的影響。本研究採用社會經濟資料服務平台提供之老人健康與社會經濟因素資料進行橫斷面研究，並依文獻回顧結果納入公園、運動中心、圖書館、捷運出入口、公車站牌、急救醫院及日間照顧中心等7種設施共同討論。 分別以路網距離、旅行時間及500公尺步行可及範圍內有無該設施等3種模型進行實證，研究發現，當限縮設施利用範圍時，模型解釋能力會微幅提升，推測係受到老人體力及移動意願影響。在老人步行可及模型中，捷運和醫院等2項設施對該里老人行動健康比率皆轉為顯著影響，該研究結果也凸顯醫院可及性對行動健康之作用並非無遠弗屆。而社會經濟變數雖不是本文研究重點，但老人經濟狀況在3種模型皆一致顯示，與老人行動健康之關聯性達到1%顯著水準。 本文根據實證結果提出政策建議，考量經濟因素在3項模型中皆對老人行動健康表現顯著解釋力，故建議未來政府規劃老人政策時，可針對長照政策前端之健康老人進行補貼、鼓勵企業聘用二次就業高齡人口，或研議放寬退休後再就業限制等方式以改善老人經濟狀況。針對捷運、醫院可及性與老人行動健康之顯著關聯，建議政府部門於都市計畫通盤檢討階段關注區域老人人口比率，邀集衛福及交通單位共同研議配置相應醫療及捷運連結系統，相信可對健康老人之在地老化有正面影響，同時紓解政府長期照顧財政負擔之壓力。
This study applies the Health Services Utilization Theory to examine the impact of aging-in-place facilities on elderly mobility health, using "accessibility" as a key factor. Unlike previous research relying on survey data, this study utilizes population data and quantitative regression analysis, with facility accessibility as a proxy variable. A cross-sectional study was conducted using data from the Social and Economic Data Service Platform, incorporating seven types of facilities: parks, sports centers, libraries, metro stations, bus stops, emergency hospitals, and day care centers. Three models were tested based on network distance, travel time, and presence within a 500-meter walking range. Findings show that restricting facility utilization range slightly increases model explanatory power. In the walking accessibility model, metro stations and hospitals significantly affect elderly mobility health ratios in their neighborhoods. Economic status consistently shows a significant association with mobility health across all models. Policy recommendations include subsidizing healthy seniors, encouraging employment of older workers, and relaxing post-retirement employment restrictions to improve elderly economic conditions. For urban planning, it is suggested to consider the proportion of elderly population and involve health, welfare, and transportation units in configuring medical and metro systems.
