An Investigation on the Responsiveness of Immunity in the Immune Therapy of Hiv Infection - a Mathematical Perspective

摘要 愛滋病是由人類免疫缺陷病毒所引起,在感染末期會使人免疫功能缺陷而因其它感染死亡,根據統 計,至2012 年為止,愛滋病已造成三千六百萬人死亡。由臨床觀察顯示,有少數人經此病毒感染後, 體內的病毒量可持續保持在相當低的水平,有更少數的人經藥物治療後,可處於免疫有效控制體內病 毒量的狀態,這些臨床觀察顯示利用患者的免疫來長期控制體內的愛滋病毒是可能的。然而,在一項 與免疫療法有關之大規模臨床實驗中,患者可能因免疫反應激發不足而陷入危險,這說明了免疫反應 率在免疫療法之重要性。在我們之前對愛滋病毒感染數學模型研究中顯示,免疫反應在某閥值前有衰 退現象是免疫療法可行之必要條件,在此條件成立下,本研究將進一步分析模型參數與免疫反應率之 關聯性,來瞭解參數所代表之生物機制如何影響免疫療法成效,其成果將促成對愛滋病的進一步瞭解 與治療策略之發展。
Acquired immune deficiency syndrome (AIDS) is caused by the infection of human immunodeficiency virus (HIV). In the late phase of the infection, it leads to the deficiency of patients’ immunity and they may die because of life-threatening opportunistic infections. According to statistics, AIDS has caused approximately 36 million deaths worldwide by 2012. In the literature, it has been reported that there exist HIV therapy-naïve infected individuals, called “elite controller,” who may maintain very low within-host virus levels. There also exist rare individuals whose immune responses can suppress and control the within-host viruses after cocktail therapy is initiated during the acute infection or chronic infection. These observations imply that a long term control of within-host virus with hosts’ own immune responses is possible. However, in a large scale cohort study relevant to immune therapy, it shows that patients with insufficient immune responses could be in great danger during the treatment process. This indicates the importance of the responsiveness of immunity in this type of therapy. In our prior study, a necessary condition for the feasibility of an immune therapy in an HIV infection model has been derived. The condition shows that an immune therapy is possible only if patients’ immune response attenuates below a threshold. Under such a condition, this study proposes to further explore the relationship between model parameters and responsiveness of the immunity. It seeks to understand how biological mechanisms representing by these model parameters affect the effectiveness of the immune therapy. The results will enhance the understanding to AIDS and will also lead to the development of better treatment methods of this disease.
