摘要 | 癌症已連續32年高居國人死因首之首,然而隨著醫療的提升,癌症的存活率也顯著上升,因此除癌症本身的預防與治療外,相關症狀與治療過程對於生活品質的影響也逐漸受到重視,睡眠困擾是干擾患者生活品質最常見的症狀之一,失眠認知行為治療(cognitive behavioral therapy for insomnia, CBT-I)對癌症相關失眠的療效也得到研究支持,然而過去研究多直接套用慢性失眠的概念,未能考量疾病特性與治療的影響。本計劃擬透過三個研究,探討癌症相關失眠的認知行為相關因素,並發展特定的預防及治療方案。研究一將先針對癌症相關失眠發展認知及行為因子的測量工具,並探討這些因子與不同治療與恢復階段癌症患者睡眠困擾的關聯性,以建立癌症相關失眠的病因模式,並作為介入方案設計的參考。研究二將根據癌症患者的特性,發展預防性的睡眠衛教方案,先以乳癌患者為對象,在開始進行化療時進行衛教,內容包含睡眠衛教、罹癌的心理壓力與治療過程可能帶來的睡眠干擾,以及失眠的認知行為病因,並追蹤此方案對於慢性失眠的預防效果。研究三則針對初期出現睡眠困擾的乳癌患者發展短期CBT-I介入方案,預計透過癌症相關的睡眠衛教、CBT-I的認知與行為技巧,以及放鬆訓練,探討其對失眠症狀的改善程度,以及避免睡眠問題的慢性化的效果。 Cancer continues to be the leading cause of death in Taiwan for the past 32 years. However, due to the improvement of cancer treatments, the survival rate from cancer is increasing. Thus, in addition to the prevention and treatment of cancer per se, the management of associated symptoms and psychological impacts has become an important aspect for cancer treatment. Among the associated symptoms, sleep disturbance can have great impact on the quality of life in cancer patients. Cognitive behavioral therapy for insomnia (CBT-I) has also been proven to be effective in treating cancer-related insomnia. However, most previous studies adopted the etiological and/or treatment models for chronic insomnia directly and did not consider specific conditions in the context of cancer. Cancer-related insomnia is often precipitated by both the psychological and physiological factors associated with the diagnosis and treatment of cancer. The current proposal includes three studies designed to investigate the cognitive and behavioral factors associated with cancer-related insomnia and treatment strategies. Study 1 will develop rating scales for cancer-related sleep cognition and behaviors, and investigate the role of these factor in the sleep disturbance across different stages of cancer treatment and survival. The results will be applied for the establishment of etiological model and the development of intervention programs for cancer-related insomnia. Study 2 will develop a preventive sleep hygiene education program based on the specific features of cancer-related insomnia and the findings of Study 1. The program will be administered to breast cancer patients in the beginning phase of chemotherapy. The contents of the program will include general sleep education (e.g. mechanism of normal sleep regulation, etiological model for the development of insomnia) as well as cancer-specific sleep education (e.g. psychological and physiological influences of diagnosis and treatment of cancer on sleep). The preventive effect of the program will be evaluated after active cancer treatments are completed and at 6- and 12-month follow-ups. Study 3 will develop a brief CBT-I for newly developed insomnia in breast cancer patients. The treatment will include general and cancer-specific sleep education, behavioral and cognitive techniques of CBT-I, and relaxation training. The effect of the program in reducing insomnia symptom and preventing the development of chronic insomnia will be examined after treatment and at 6- and 12-month follow-ups. |
