摘要 氨基丁酸以及麩胺酸是大腦主要的抑制性及刺激性神經傳導物質,而MEGA-PRESS 磁共振頻譜技術是目前唯一以非侵入式同時量測活體大腦這兩種神經傳導物質的方法,目前已應用在觀測多種神經及精神異常疾病之病人的濃度變化,也被應用在探討神經傳導物質和大腦活動間的相關性,但MEGA-PRESS 為單點量測技術,單次掃描僅能得到大腦單一區域的濃度資訊,在應用上有諸多限制。本計畫旨在結合MEGA-PRESS 與快速磁共振頻譜影像技術(MEGA-PEPSI),建立適用於一般研究以及臨床使用的掃描方法,用來量測大腦氨基丁酸以及麩胺酸的濃度分布影像,本研究預計使用快速面回訊影像技術,可在10 鐘內量測大腦單一切面的神經傳導物質濃度分布影像,具備1 cm3 的空間解析度,此技術將於仿體測試,並於健康受試者驗證其可行性,我們將利用不通切面方向,用來涵蓋許多不同的大腦區域,建立量測這些重要大腦區域量測的重現性指標,最後並測試利用方法達到測試高解析度濃度影像的可行性(0.24 cm3)。本計畫所完成的氨基丁酸以及麩胺酸濃度影像量測技術,將可適用於各種生物醫學研究甚至臨床研究中,對於已大量使用MEGA-PRESS 方法來量測活體氨基丁酸以及麩胺酸的神經科學研究及精神異常疾病研究而言,利用MEGA-PEPSI 直接量測不同腦區的濃度分布,是應用上的一大突破。
-aminobutyric acid (GABA) and Glutamate as the major inhibitory and excitatory neurotransmitter in the brain plays an important role in the mechanism of neurotransmission. MEGA-PRESS as one kind of magnetic resonance spectroscopy (MRS) methods is the only non-invasive method able to measure in vivo GABA concentrations without exposure to ionizing radiation. This method has been applied to detect change of GABA and Glx, a combination of Glutamate and Glutamine for a variety of neurological and psychiatric disorders. It has also been used to investigate the relation between GABA level and brain activity. The major limitation of MEGA-PRESS is that it is single voxel based method, in which information is acquired from only one brain regions at one time. This project aims to combine MEGA-PRESS with fast Magnetic resonance spectroscopic imaging method (MEGAPEPSI) to measure the spatial distribution of GABA and Glu over a slice. The established protocol can achieve 1cm3 spatial resolution with scan time less than 10 minutes. The protocol will be examined on custom-made spectroscopic phantom and examined on healthy subject for feasibility. We will use MEGA-PEPSI in transverse and sagittal orientation to cover various brains of interest and establish the reproducibility of GABA and Glu quantification for these brain regions. We will further examine the feasibility of high resolution MEGA-PEPSI with spatial resolution up to 0.24 cm3. T The project has contribution to medical researches and potentially clinical applications. The developed MEGA-PEPSI method can simultaneously measure important neurotransmitter, GABA and Glx, over various brain regions. The information can serve as important index in neurological and psychiatric disorders, and can be combined with finding from neuroscience to understand the brain functions.
