題名 癌後正向成長、發展軌跡及其影響因子與身心健康的關係 (III)

摘要 本計畫整理過去有關探討創傷後成長與身心適應的關係,發現研究結果不一致。之所以如此,本計畫認為創傷後成長可能具有兩個成分,一為真實的成長,二為正向錯覺;在測量上,使用益處發現不足以代表創傷後成長;在概念上,本計畫認為癌後的正向成長可能具有不同的發展軌跡。因此,本計畫採用診斷後、術後三個月、術後六個月、術後一年及術後兩年的長期縱貫研究探討150 位乳癌患者與150 位大腸直腸癌患者的癌後成長軌跡,如能預期得到不同組別,將比較這些不同組別在個人背景變項、臨床變項、對癌症的壓力與威脅知覺、知覺癌後的益處與傷害性、因應、審思及反芻、防衛性、自尊與身心適應指標的差異。另外,本計畫嘗試進行壓力處理的認知行為團體治療及書寫表達,以說明心理介入是否可增加癌症患者的正向成長。
This proposal reviewed the topic of relationship of posttraumatic growth and psycho-physical adjustment, found the inconsistent results. This proposal thinks the factors that influence the above results are: 1. Posttraumatic growth maybe existed two components: the real and the positive illusion; 2. In measuring the posttraumatic growth(PTG), the Benefit Finding Scale could not represent the construct of PTG; 3. There maybe exist different developmental trajectories of positive growth in the aftermath of cancer. So, this proposal uses the longitudinal study and five times of measurement to detect the different developmental trajectories of positive growth. If the groups of different trajectory could be found, this proposal will compare these groups in demographic variables, clinical variables, perceived stress and threat in cancer, perceived benefits and vulnerabilities in cancer, coping, reflection and rumination, defensiveness, self-esteem and psycho-physical adjustment. This proposal also intervenes with cognitive-behavioral stress management (CBSM) group intervention and expressive disclosure to improve the growth of cancer patients.
